Eduardo J. Guimbarda, MHA, MBA
President & CEO
Eduardo’s 30 years’ experience, in home health and post-acute care services, prompted the creation of VigeoDash as an efficient and impactful solution to industry challenges. As a healthcare strategist/consultant, Eduardo knows what truly covers the gaps in the continuum of care and what drives patient adherence and outcomes during all transitions of care. Eduardo’s vision is to connect healthcare providers through systems and technology to reduce costs and improve outcomes. In addition to co-creator of VigeoDash, Eduardo is a healthcare strategist for Healthcare Integrative Solutions and CEO of Home Nursing & Therapy Services, which has served the community for nearly 30 years. Eduardo recognizes what covers the gaps in the continuum of care, drives patient adherence and outcomes during all transitions of care.
Eduardo has successfully implemented independent programs that improve transitions of care. He is currently implementing a full-service Transitional Care Model for one of the largest hospital systems in the U.S. The program is elaborate, unique and is in process for approval as best practices and gold standard of care. The model also includes long-term population health programs; chronic care management with a collaborative behavioral health program focusing on social determinants of health (SDOH). He is working on a business case for CMS Center for Medicare & Medicaid Innovation to improve future program development. In addition to his extensive experience, Eduardo is a trained program implementation coach.
Eduardo holds a Bachelor of Arts in Psychology from St. Mary’s University, a Master’s in Business Administration from the University of Texas in San Antonio and Master’s in Healthcare Administration from Cornell University. His capstone thesis for the Sloan Program, Cornell University gained statistical validity on his collaborative transitional care program with tele-health inclusion as a successful process innovation for CMS. Eduardo is married, father to 7 children and is a current resident of San Antonio, Texas.
Nagarajan Sankrithi, PhD, MHA
Chief Information Officer
In addition to his role, as CIO, with VigeoDash, Nagarajan is a healthcare strategist/ consultant for Healthcare Integrative Solutions, LLC. He brings his extensive expertise in public health, pharmaceutical informatics to the healthcare space. For the past 17 years, he has built and delivered appropriate informatics solutions to transform research operations for early drug-discovery to pre-clinical research for Abbott/AbbVie, Merck and Massachusetts Public health Laboratory. His informatics career blossomed while he was working as an assay development scientist for a central nervous system (CNS) drug discovery start-up pharmaceutical company EnVivo (Forum). He built the informatics solutions that was instrumental in automating a high throughput behavioral assay data platform and established a cheminformatics data science backbone. He used it to select the clinical candidate for Alzheimer’s/ Schizophrenia (EVP-6124/Encenicline).
Nagarajan consulted at AstraZeneca where he helped the Antibody and Protein Engineering Group to develop an informatics solution for the proteomics, metabolomics and lipidomics lab. While pursuing the pharmaceutical research consulting, he is co-implementing a full-service Transitional Care Program.
Nagarajan holds a Bachelor’s in Biology and Chemistry from University of Wisconsin- Superior, a Master’s in Biochemistry from University of Houston, a PhD in Neurobiology from Northeastern University and a Master’s in Healthcare Administration from Cornell University. His Capstone thesis for this program focused on a comparative analysis of operational efficiencies of an independent homecare agency (HNTS) and a large hospital-based homecare and hospice business (EverGreenHealth). He is currently developing clinical dashboards that will assist homecare agencies with operational efficiencies, revenue cycle management improving overall financial health.
Shawn Woolard, RN
Clinical Consultant
Shawn is a great fit for the VigeoDash team with home health care experience starting in 2008. Shawn began his nursing career as a PICU Nurse, and then as a Travel Nurse. Shawn is the Director of Clinical Services for Home Nursing & Therapy Services. Additionally, Shawn has an abundance of experience in post-acute care and oversees the clinical operations of the home health agency.
Shawn earned his Associates Degree of Nursing from Amarillo College and is currently a multi-state licensed registered nurse in the state of Texas.
Aanika Rahman
Lead Software Solutions Developer
Aanika serves as the Lead Software Solutions Developer at Healthcare Integrative Solutions, seamlessly undertaking the roles of a solutions architect, data engineer, and tech lead.
In her multifaceted role, Aanika leverages the broad experience she acquired from various startups and previous collaborations with clients in the financial, technology, health, and energy sectors. She established the groundwork for her career by earning a Bachelor of Science in Mathematics and Computer Science from McGill University. Her technical expertise now spans data analytics, data visualization, fullstack development, and machine learning.
Dedicated to fostering learning and community engagement, Aanika co-chaired the Canadian University Software Engineering Conference, attracting 500+ students nationwide. She also participated in and coordinated the AI4Good Lab, demonstrating her commitment to leveraging technology for social good. Beyond the tech realm, Aanika has also made meaningful impact by working closely with children with disabilities in Bermuda and Montreal.
Fueled by a passion for both technology and community, Aanika stands as a driving force in Healthcare Integrative Solutions. Embracing challenges with a strategic mindset, Aanika continues to strive for innovative software solutions that redefine the future of healthcare.
Sowmya Ravi
Business Analyst/Product Manager
Sowmya's role at Healthcare Integrative Solutions is pivotal, involving leadership in multifaceted projects, identification of process optimization opportunities, resource coordination, and ensuring timely delivery. Her journey into healthcare began with an undergraduate degree in dentistry, providing her with a robust foundation in patient care and medical sciences. Intrigued by the broader landscape of healthcare, she pursued a Master's in Health Administration from the Sloan Program at Cornell University, refining her skills in management and strategy.
With a keen eye for detail and a passion for the future of healthcare, Sowmya has ventured into various roles, including serving as a Business Process Manager at a tech company. Her diverse experiences culminated in a transformative internship with CVS Health, where she contributed to a groundbreaking platform aimed at incorporating clinical decision support for Kidney Care patients.
Motivated by a commitment to leverage technology for improved patient outcomes, Sowmya is currently dedicated to exploring predictive analytics and AI solutions through VigeoDash.
Beyond her professional pursuits, she holds a deep appreciation for the arts and holistic well-being. Trained in Indian classical dance, she has captivated audiences across India, the US, and Europe with her graceful performances. When not immersed in healthcare innovation, Sowmya enjoys experimenting in the kitchen, mastering new yoga poses, and nurturing her love for cats through fostering.
